Preschool Summer Information

We are honored that you have chosen Zeeland Christian Preschool, and we look forward to a great year of learning and growing together! We hope the information included here will be helpful to you as you prepare for your child’s school experience.

Preschool Summer Student Paperwork

There are several preschool forms that we ask you to complete and return by July 24. This early deadline enables us to focus on you and your child in the first days of school instead of worrying about papers and information. Children will not be able to attend preschool classes until all paperwork is completed.

  • Every child is required to have a new Child Information Record each year.

  • This form contains all contact and emergency information necessary for the child.

  • Each space on the form is required to be filled in, even if the line includes “same”, “none”, or “not applicable” unless it is listed as optional.

  • Only the people listed on the bottom of the form may pick up your child from school.

  • For the Parent/Legal Guardian Section - Please initial and write Zeeland Christian Preschool in the permission section.

  • This form must have a signature on the bottom to be complete.

• • Preschool Health Appraisal Form (click to print a PDF)

    • Every child is required to have a health appraisal on file.

    • State law also requires that every child must be current on his/her immunizations or have a waiver from the health department on file at the school.  We received information from the Ottawa County Health Department that they are short-staffed this summer, so if you need to set up an appointment, please do so during July.

    • Make an appointment with your health care provider between now and August to update immunizations and complete the health appraisal.  If your child’s appointment is after the July 24 paperwork deadline but before school starts, please email Carrie Papay, at cpapay@zcs.org, to let her know the date of the appointment so we know when this form will be turned in.

    • If your child has had a well-child visit in the last 6 months, your provider may be able to fill out the form without an appointment.

    • Note: If your child has already attended our preschool, you received information during the last week of school regarding his/her health form.  We either kept it at school because it was valid for another year, or we sent it home to you so you could obtain a new, valid health appraisal.

  • Please complete and return with health appraisal.

• Zeeland Christian Preschool Online Form (Click to complete and submit online)

  • This Online Form includes information that we either need to know for licensing purposes or that will help us prepare for your child’s school year.

  • It takes about 10 minutes to complete, but you cannot save your answers partway through, so complete it all at once and hit “submit”.

Tuition Payments
All families should be registered for an account through SMART Tuition. Through SMART Tuition, you have the option to pay annually, semi-annually, quarterly or monthly.

If you chose to pay annually or semi-annually, your first payment will be due on July 5 or 20 (whichever date you chose in SMART Tuition).  You will receive an emailed invoice, and you may choose to pay via credit card, bank account deduction, mail a check to SMART Tuition, or mail/bring a check to school.

If you chose to pay quarterly or monthly, your first payment will be deducted from your bank account or credit card (whichever you chose in SMART Tuition) on July 5 or 20 (whichever date you chose in SMART Tuition). Monthly payments will be distributed over 12 months (July 2019 - June 2020).  Quarterly payments are due in July, October, January, and April.

Returning Preschool Paperwork

We encourage you to mail your completed Child Information Record, Preschool Health Appraisal Form Consent for disclosure form, and Tuition Payment (if paid annually or semi-annually) any time before July 24: Zeeland Christian Preschool, 334 W Central Ave  Attn: Preschool Forms, Zeeland, MI 49464. You may also email completed PDFs or scans to registration@zcs.org. If you choose to bring your paperwork to school, the main office is open from 9:00-1:00, Mondays-Thursdays, throughout the summer (except July 1-4). Preschool Class Lists

You will receive a separate email with an attached PDF class list for your preschooler.  Please review your information, and email registration@zcs.org if there are any changes.
